Looks like Historical Atlas by William R. Shepherd has most of what you need. It's available on the Internet Archive, but the online copy available at the University of Texas website might be more convenient: Historical Atlas by William R. You'll probably want the Ecclesiastical Map of Western Europe in the Middle Ages.
There's also the Digital Atlas of Dioceses and Ecclesiastical Provinces in Late Medieval Europe (1200-1500) which is more precise, but more unwieldy for me personally.
