Is there a GM screen available for UA? If not, and I make my own, what information should I include?

I can easily answer the first part of your question -- No, there's not a GM screen for UA.
but is there any possibility that You will produce one?
Unknown Armies has been out of print for quite some time. While there is talk that there might be a reboot of the system in 2015, I doubt we'd be seeing an official GM screen either way - unfortunately they're niche products and a tough sell.
To include the OP's second question: I'd not even be sure what to put on a GM screen for Unknown Armies - there's not that many hard mechanical bits to be referenced.
