Yes, still very USA-centric, but still playable in other parts of the wide, weird world.
My group has been playing 3E with the cabal based in Germany. As GM, I've had to adapt a few things, but still quite doable.

Followup question:
Is the setting still the same (TNI, Archetype ascension, St G, the Porn Goddess, Cliomancy) or are there changes - new factions, new magicks, etc?

There are callbacks to the original factions and magick (particularly in Books 4 and 5, which were Kickstarter stretch goals), but in updating the setting, Greg and his team focused on providing new content for players to use and explore. So the answer is really both—the core rules give you tons of new stuff, but because fan demand for classic archetypes and magick was so high, much of that will be available as well.
